1. Request by School District to consider referral. A determination is made as to whether student is a potential applicant; does he/she have approved coding for program?
  2. Initial intake meeting scheduled to include tour of school. This may be just parent, parent and district or parent/district/child and/or collateral contacts.
  3. Releases of information forms are signed and paperwork is gathered for review.
  4. Information gathering with the district, parent and/or other significant school/community personnel.
  5. If felt that the student may be an option for placement and parent/district agree, a student visitation is scheduled for half or full day, depending on the program.
  6. A final decision will be made after the team has met and contact with collaterals has been completed.
  7. Upon acceptance, a placement meeting is scheduled and an admission date established. A 30 day evaluation and/or IEP meeting is scheduled.
